Current Illness Protocols
Cold and Flu Season
Cold and flu season are here. The flu is a respiratory disease and not a stomach or gastrointestinal illness. While the flu and common cold are both respiratory illnesses the flu is generally worse than the common cold (typically runny and stuffy noses). The flu involves symptoms such as fever, body aches, fatigue, sore throat, and a dry cough.
The best way to protect oneself from the flu is to get a flu shot. The CDC recommends that everyone six months and older should get a flu shot. You should get vaccinated as soon as the vaccine is available. However, you can get immunization any time throughout flu season. Getting the vaccine later in the season (December – March) can help protect you from getting the flu. The following are good practices to prevent the flu whether or not you are able to get vaccinated:
- Avoid close contact.
- Avoid close contact with people who are sick. When you are sick, keep your distance from others to protect them from getting sick too.
- Stay home when sick.
- If your child has a temperature of 100° and higher or exhibits flu symptoms, it is best to keep him/her home from school. If possible, adults should stay home from work. You will help prevent others from catching your illness.
- Cover your mouth and nose.
- Cover your mouth and nose with a tissue when coughing or sneezing. Or, cough and sneeze into your elbow and not your hands. It may prevent those around you from getting sick.
- Wash your hands.
- Washing your hands often will help protect you from germs. Please encourage your children to wash their hands thoroughly when appropriate.
- Avoid touching your eyes, nose, or mouth.
- Germs are spread when a person touches something that is contaminated with germs and then touches his or her eyes, nose, or mouth (i.e. door knob). Most healthy individuals may be able to infect others beginning 1 day before they have symptoms and up to 5 days after becoming sick. You can pass on the flu virus before you know you are sick.
- Other good habits such as getting plenty of sleep, engaging in physical activity, managing stress, drinking water, and eating healthy food, will help you and your family stay healthy. If you or your family experience symptoms that you think might be the flu, you should contact your health care provider as soon as possible.
2024/2025 COVID PROTOCOLSThere are no longer quarantine protocols in place in NJ Schools. If you have tested positive for COVID, please follow the same rules and guidelines listed above to help prevent the spread. You may return to school or work once you are fever-free for 24 hours without use of fever-reducing medication and/or you are symptom-free and feel well enough to return. Family members should follow the same protocol and monitor symptoms. Schools are not required to contact trace and there are no mandated timeframes for absences.

Emergency Contacts
Please submit at least two emergency phone numbers who live within 10 to 15 minutes of the school on your child’s registration form in the event that you are not available to come pick your child up if they are ill. It is important to keep the school informed of any changes in emergency contact numbers to ensure you can be reached.
Please add the school's main number to your contacts 856-869-7750. This will avoid having "unknown number" come up on your phone.
Asthma Action Plan/Printable Forms
If your child requires an inhaler or nebulizer medication to be kept at school, please have your child's doctor fill out the Asthma Action Plan Form
**Please make sure the parent/legal guardian signs the form in order to give consent to administer the medication.**
If your child has permission from their doctor to SELF-CARRY their inhaler; please be sure they have it with them at all times times
Epi-Pen/Allergy Information: Printable Forms
I strive to keep all of our students safe while at school and allergies are taken very seriously. Please take time to review the following form if your child has a life threatening allergy.
Please have your child's doctor complete the order form below for an epinephrine auto-injector (Epi-Pen) for use in the event of anaphylaxis. Also have them include the dosage of Benedryl if they would like your child to receive that as well. Please make sure the parent/legal guardian signs the form in order to give consent to administer the medication. *your doctor may give permission for your child to self-carry their medication. Please be sure that they indicate on the form if this permission is granted*
Medication Administration/Printable Forms
It is important that I am aware of any health changes or chronic conditions that pertain to your child so that I may treat and care for them safely and effectively. Please notify me of any new illnesses, injuries, or treatments.
If medication MUST be given to your child during school hours, the requirements are as follows:
- Medication MUST be brought in by an adult
- Medication must be in properly labeled, original bottles
- A doctor's note/order AND parental consent must be given to the nurse

OVER THE COUNTER MEDICATION ADMINISTRATION FORMS
Please click the button below to download and print the HT Parent Permission form for OTC medication administration. Nurses are permitted to administer TWO doses of OTC medication to students with a signed form on file. Only hard copies will be accepted. No emailed or verbal permission will be accepted. This permission is good for one school year and must be updated each year. If your child requires more than two doses per week, or a non-standard dose of medication, you will need to hand in signed doctors orders for school medication administration. Below you will find printable permission forms for Tylenol/Ibuprofen and TUMS administration. Your child must have a signed permission form on file for the current school year in order to receive any medication in school.
Required 6th Grade Vaccines
Dear 6th Grade Parents,
Please read the following carefully for VERY IMPORTANT information regarding vaccinations required for all incoming 6th graders. We understand that there may be many variables in place when it comes to immunization schedules and obtaining records. The FAQ will answer all questions you may have and contains information regarding how to get vaccines free of charge or without affecting your annual well-visit schedule.
Please be advised that The New Jersey Department of Health requires that all 11-year-old students entering 6th grade receive the TDap and Menactra vaccines. Please send in documentation of your child's vaccines so that we can keep our records accurate and remain in compliance with the NJDOH . If your child is not yet 11 by the first day of school, proof of vaccination within two weeks of their eleventh birthday is required. Thank you for your help in this matter and as always, please reach out if you have any questions.

FAQ 6th Grade Vaccines
What vaccinations are required for all incoming 6th graders in NJ?
- TDap and Menactra (Tetanus Booster and Meningitis)
What if my child is not yet 11 years old when school starts?
- If your child is not yet 11, you must provide proof of vaccination to the school within 2 weeks of their 11th birthday
What if I have a religious or medical exemption?
- You may provide written documentation to the school expressing religious exemption for vaccination. If your child has a documented medical exemption, please provide a letter from your physician stating the exemption.
What if my child's well-visit isn't scheduled until after school starts?
- You may make a "vaccine only" appointment with your provider. Some practices call this a "nurse visit". It will not affect your yearly well visit coverage with your health insurance
What if I do not have insurance or an established medical provider for my child?
- Camden County provides vaccination services free of charge for those who are uninsured. You must make an appointment in advance by calling 856-225-5128 for the Camden location or 856-931-2700 for the Bellmawr Location. Some retail drug stores also provide these vaccines. Check websites for local stores for information
What will happen if my child does not have vaccines on file when the State audits RMS?
- The state of NJ will require that your child be excluded from school until documentation of vaccination is received. The state may come to audit our school at any time within the first 30 days of school beginning.
When will I be notified of this vaccination policy?
- Informational letters go home with each 5th grade student in January so that ample time is provided for appointments and arrangements to be made. It is our goal to be in full compliance with the law so that none of our students are excluded at RMS. This website is available for reference and reminder emails will be sent home. Information will be presented in person to parents at 6th grade orientation as well.
What is the deadline for providing proof of vaccination to the school?
- The law states that students should provide proof of vaccination or proof of appointment for vaccination by the first day of school. The only exceptions to this are those with documented religious or medical exemptions and those who are not yet 11 years old on the first day of school. If your child is 11 years old, they may receive vaccines at a vaccine only appointment with your provider and this will not affect your yearly insurance coverage for well-visits. If you are attending HT schools for the first time and are coming from another state or country, the state provides a 30 day grace period to show proof of vaccination.
Can I send in a screenshot of my child's vaccine records?
- No. Please provide hard copies of vaccine records. Digital copies don't always print out in a legible way and it's important that we be able to see the dates of vaccine clearly. These records remain in your child's medical file throughout their school career. You may drop them off at any time to the RMS main office. Staff is available throughout the summer during normal school hours.
- **PLEASE DO NOT SEND YOUR VACCINE RECORDS TO YOUR 5TH GRADE NURSE. PLEASE SEND THEM DIRECTLY TO RMS**
Sports Physicals/Printable Forms
Sports Physical Policy
Each student athlete who wishes to participate on a middle school athletic team or non-competitive club must have a physical exam within 365 days prior to the first day of practice. Sport Physicals are only valid for one year from the date of examination by a phyician.
Sport Physicals must be recorded on New Jersey Department of Education approved physical forms. NO OTHER PHYSICAL FORMS WILL BE ACCEPTED.
- Submit all completed Sport Physicals to the School Nurse. Completed packets may also be placed in the nurse's mailbox located in the Main Office at any time. Hard Copies are the only form of documentation that will be accepted. Please do not email or scan/screenshot your physical paperwork
No student will be allowed to practice or participate in any sport without a current, valid physical exam on file in the RMS Health Office. Paper work packets are always available during the summer months in the Main Office and throughout the school year in the RMS Nurse's Office or may be downloaded from the district web site.

What Sports Require a Sports Physical at RMS?
All sport-related teams and clubs require a valid, up to date sports physical to be filled out and handed in to the nurse prior to participation.

Where Can I Find the Sports Physical Paperwork:

How Often Do I Need To Hand In A Sports Physical?
Sports Physicals are valid for ONE YEAR FROM DATE OF EXAM BY PHYSICIAN. Your doctor will note the date of last exam on the form. 365 days from that date, your physical will expire. Ex: Date of Exam 9/11/23, Date of Expiration 9/11/24.
Will My Child Be Permitted To Practice or Participate If They Do Not Have a Valid Physical on File?
No, only students with valid sports physicals on file will be permitted to participate. Your child will be allowed to attend practices to observe but will not be permitted to participate until a completed sports physical is on file with the nurse.
*It is the student's responsibility to check with the nurse to find out if they need to turn in any paperwork prior to the start of the sport season*
Can I Hand In A Print out of My Child’s Annual Check From Their Doctor?
No, Sports Physicals must be filled out on the provided forms and returned to school
Can I Scan/Email My Forms to Mrs. Martin?
No, please send in hard copies/originals to Mrs. Martin so that they may be forwarded to our district physician for signature and approval. They will then be kept in your child’s medical file at school. Scanned/Faxed or screen-shotted copies are often not legible and do not print out correctly.
What If My Child's Well Visit Can't be Scheduled Until AFTER the Sport Season Starts?
You may take the Sports Physical Forms to any Urgent Care and receive a Sports Physical which will not affect your annual well-visit schedule with your healthcare provider
How long does it take to get a sports physical done?
Doctor's offices can take 7-10 days to return a completed sports physical. It is important that you give enough time for the forms to be completed by your doctor and returned to school so that your child does not miss the start of their sport or club.

Impact Testing
Impact Testing (pre-concussion assessment) is required for ALL student athletes. It is not required for students partaking in our extracurricular club teams (broomball, volleyball, tennis, bowling) Once taken, the test results are good for two years. If taken for the fall season, there is no need to re-test for the winter or spring seasons.
